⭐ 7. Family Services Referral Network

Our Purpose: Connect vulnerable families to essential health, legal, and social services by identifying their needs, guiding them to solutions, and following up to ensure.

EXPANSION: Activities

  • Identify children or caregivers in need of:

    ▪immunizations

    ▪HIV care

    ▪maternal health services

    ▪malnutrition treatment

    ▪mental health support

    ▪disability care or rehabilitation

    ▪Connect them with nearest HC II, III, or IV

    ▪Provide directions, transport support (if critical), and help fill forms

    ▪Follow up with the facility or caregiver to confirm the visit happened

    👉 In many cases, healthcare is technically free  but inaccessible due to fear, stigma, ignorance, or distance.

    CHOCU becomes the family’s trusted health companion.

    ➡️ Especially powerful for single parents, orphaned children, or families with children living with disabilities.

  • Establish formal relationships with:

    ▪District Probation Officers

    ▪Child and Family Protection Units (CFPU - Uganda Police)

    ▪Local Councils (LC1–LC3)

    ▪Gender-Based Violence Response Centers

    ▪Disability Rights Organizations

    ▪Elder and widow care programs

    ▪Co-create referral pathways with memoranda of understanding (MOUs)

    ▪Hold joint community sensitization events on rights and services

    👉 Most families don’t know what help is available or how to access it.

    CHOCU acts as a bridge  walking with the family, not just pointing the way.

  • Act immediately in urgent cases such as:

    ▪Domestic violence

    ▪Child abandonment

    ▪Sudden homelessness

    ▪Teen pregnancy rejection

    ▪Family death or house fire

    ▪Call appropriate authorities and activate emergency aid (food, clothing, shelter)

    ▪Provide emotional support while professional services respond

    👉 These moments define CHOCU’s moral authority in the community.

    Even without money, your rapid response saves lives and builds trust.

    ➡️ Crisis work positions CHOCU as a community protector, not just a provider.

  • After any referral or crisis, CHOCU staff or volunteers return to:

    ▪confirm the service was received

    ▪check on emotional recovery

    ▪offer encouragement or basic material support

    ▪note unresolved needs

    ▪Document status updates with basic case notes

    ▪Maintain confidentiality and dignity

    👉 Follow-up is what separates CHOCU from others. It proves you care what happens after Day One  not just in the moment of attention.

    ▪Avoid delays that cause medical deterioration, legal escalation, or emotional trauma

    ▪Reduce time children spend in unsafe or unstable situations

    ▪Families begin trusting systems again
